Laparoscopic Resection of a Large Gastric Duplication Cyst in an Adult

Abstract

|
A gastric duplication cyst is an extremely rare developmental anomaly that comprises 2.8% of all alimentary duplication cysts with most of these being reported in children. Moreover, the possibility of a malignancy within it should be considered, and particularly in adults. Because of this, complete excision of duplication cysts is important. Laparoscopic gastric surgery has become feasible with the recent development of skills and instruments. We report here on a 17-year-old female who had postprandial abdominal discomfort. One cystic lesion was found by abdominal computed tomography (CT) in her stomach at the cardia region preoperatively. Laparoscopic wedge resections of the gastric lesions were performed. We used two 12 mm trocars and two 5 mm trocars. We resected the gastric duplication cyst with an endo-GIA and harmonic scalpel. We closed the esophagogastric junction region with hand sewing for prevention of narrowing. After resection, we inserted a closed suction drain via a 5 mm trocar at the left upper quadrant of the abdomen. The postoperative pathologic examination disclosed gastric duplication cysts with no evidence of malignancy. The patient made an uneventful postoperative recovery and has remained asymptomatic for 12 months after surgery. A laparoscopic resection of a duplicated stomach is less invasive and esthetically superior to other methods, and so it is considered to be a useful therapeutic modality.
